Feature request: Headers & code snippets

A major upgrade would be to add the other 4 header options (h3-h6), since the whole point of using a visual editor is to enable someone who doesn't know how to write markdown to do so with ease. It seems like a simple inclusion, as well as code blocks. Those are the two main things keeping this plugin from being a complete markdown editor that it seems like an obvious next step.
