switching back to call on hold fails

Steps to reproduce the bug
- Start a phone call
- Click on the "add call" button
- Call a second number but don't merge the call (first call is on hold)
- Have call number 2 hang up on you
Expected behavior
You return to the first call as usual.
Actual behavior
The active call doesn't respond. You can see the name and picture of the contact from the first call, but you can't hear them (and they can't hear you), no call controls are present. Even after going back to the home screen and back to the call this still happens. The only way to end the call is to hang up from button on the call notification.
