Create "Claim Your Badge" page

As a non-FP user
- As I land on the "Claim Your Badge" page, in order to claim my specific FP/DG badge, I should be prompt to do the following:
- Input my EMAIL ADDRESS
- Include free minting in Matic
- Once I input my email address, and select or not select free minting in Matic, I should receive an email to signup for an FP wallet and a confirmation of submitting my email.
*Keep in mind, the link that non-FP-account holders receive is connected to their specific badge.

Acceptance Criteria:
Users should be able to do the following:
Input only 1 EMAIL ADDRESS within the claim your badge page.
The email address field should only recognize email addresses.
