[topicmappr] nitpick: clarify that `.json` extension is not needed for `--out-file`

Noticed that .json is appended automatically to the value of --out-file. I had already specified it so I ended up with combined.jsons.json. Nitpick: don't append it if present, and update the doc accordingly.
