Mouse-Lock does not work properly

Summary
In some instances, the mouse will not correctly lock to the window, or will temporarily lock to the window only to unlock on say, a right-click or a loading screen. This has happened in a few games in the past but the most relevant one to me is the game Exanima, since it's the most recent.
This is only really an issue if you have say, multiple screens or you play the game in windowed mode instead of borderless fullscreen.
Steps to reproduce
- Lock mouse
- Loading screen/ingame action
- Mouse no longer locked.
