Submit Application - Email Template not correct

Issue: Submit Application- Email to user after submission is not correct
Steps to reproduce:
- Submit your TDM Points application
- Check email that arrives in your inbox
- Email is from Matthew.Crist@Boston.gov
- The information in the email is not correct
Expected results:
User should receive confirmation email after submission documenting what they submitted to BTD on the TDM points application
