Issue: Submit Application- Email to user after submission is not correct

Steps to reproduce:

1. Submit your TDM Points application
2. Check email that arrives in your inbox
3. Email is from [e-mail]
4. The information in the email is not correct

Expected results:
User should receive confirmation email after submission documenting what they submitted to BTD on the TDM points application
