Add description/Feature how to run in using self-hosted instance

Hi,

How can I use the vscode with a self-hosted instance?
Is it possible to declare the hosting URL (either localhost or something else)?

I'd wish to have this feature and if already available a description on the readme how to change the settings.
This goes along with #9 to add settings, declaring the URL
