Create an inviter for ERG and affinity group channels

From @thebestsophist in Slack, it would be handy to have a way for people to privately request access to ERG/affinity group channels without having to figure out who the admin is, etc. Users could conceivably ask Charlie for information about affinity groups, which might trigger a response like this:
Users would click a button to request an invitation. The appropriate channel would then be notified that they'd requested access, and someone in the channel can invite them.
PR #267 is a legacy implementation. It was closed because I misunderstood the intent, but I can reopen it and get it finished up now.
